Poster 322: Blood Flow Restriction after ACL Reconstruction Does Not Accelerate Quadriceps Strengthening

چکیده
Objectives: The use of blood flow restriction (BFR) therapy has gained popularity as an adjunct rehabilitation technique following anterior cruciate ligament reconstruction (ACLR). purpose this study was to longitudinally evaluate the impact that BFR on quadriceps strength ACLR with tendon (QT) autograft in comparison traditional protocols. Methods: Patients undergoing primary QT at a single institution were retrospectively reviewed. Included patients had minimum two isometric testing sessions via electromechanical dynamometer (Biodex). Obtained measures included peak knee extension torque operative extremity and ratio (KER), which nonoperative compared. Strength compared between received during postoperative underwent without for up three measurement sessions. Demographic surgical factors including patient age surgery, sex, BMI, level competition, time from injury surgery session, regional anesthesia, tourniquet time, concomitant collected. Two group comparisons continuous categorical variables analyzed by using Mann-Whitney U chi-squared test, respectively. Statistical significance set 0.05. Results: A total 45 (26 female) 36 (13 met inclusion criteria. Mean entire cohort 19.5 years. There no statistically significant differences among demographic or cohorts. Of receiving therapy, mean number treatments 18 first measurement, 8 second measurements, 9 third measurements. Peak lower (96 Nm) than (111 session (p=0.03) but similar groups (118 Nm vs 134 Nm, p=0.1) (138 130 p =0.39) difference change KER (0.81) (0.90, p=0.02), though existed Conclusions: Sequential measurements young demonstrated improvements irrespective use. when comparing groups. Both over did not accelerate gains rehabilitation. However, starting yet final cohort, role long term risk cannot be excluded.

ACL reconstruction using quadriceps tendon.
Arthroscopic anterior cruciate ligament (ACL) reconstruction is one of the most commonly performed1 and most successful procedures in sports medicine. Various autograft choices have been used for ACL reconstruction,2 and the results in terms of stability and return to sports activities have been good.3 The ideal autograft should have optimal mechanical properties and low donor-site morbidity. aforementioned reasoning would lead one to suggest that part of the benefits observed with BFR in the absence of exercise are due to this increase in EMG activity, however, in the absence of muscle contraction, there are no changes in EMG activity. Furthermore, our unpublished data also suggests that no measurable changes occur in whole blood lactate, indicating that large changes in hydrogen i...
متن کاملBlood Flow Restriction and Its Function in Post- Operative Anterior Cruciate Ligament Reconstruction Therapy: Expert Opinion
An increasingly popular method for post-operative rehabilitation of an ACL reconstruction, as a substitute for traditionaltherapy, is blood flow restriction therapy (BFR). BFR therapy utilizes a pneumatic cuff to simulate strenuous exercisein an effort to stimulate muscle recruitment, mitigate atrophy, and promote hypertrophy in patients with load-bearinglimitations. Because t...
متن کاملPostoperative radiographs after ACL reconstruction are not cost-effective.
Postoperative radiographs have become standard after anterior cruciate ligament (ACL) reconstruction, even though few findings in the literature substantiate their efficacy. The goal of this study was to determine whether routine postoperative radiographs were cost-effective in patients undergoing ACL reconstruction. In addition, the authors sought to determine whether these radiographs provide...
متن کاملDeficits in Quadriceps Strength and Patient-Oriented Outcomes at Return to Activity After ACL Reconstruction
CONTEXT Side-to-side quadriceps strength deficits are linked to hazardous lower extremity mechanics and reduced function at a time when individuals are returned to activity after anterior cruciate ligament (ACL) reconstruction. As a result, generalized criteria have emerged in the literature, wherein researchers are recommending that patients be cleared for participation once side-to-side diffe...
